Breast enlargement: methods & costs
At HANSEATICUM Hamburg, we offer a variety of breast enlargement methods to meet the individual needs and aesthetic goals of our patients. The techniques available include breast enlargement with state-of-the-art implants such as Motiva® implants and B-Lite. Another method is breast augmentation with autologous fat, popular with patients who want a subtle enlargement without foreign material. For minimal scarring, we use the minimal scar technique for excellent results.
As one of the first clinics in Germany, we have also recently added the innovative and particularly gentle method of breast harmonization with Mia® Femtech implants to our portfolio.
These methods differ not only in terms of surgical technique and aesthetic results, but also in terms of associated costs. For example, breast augmentation with silicone implants costs from €7,450 including VAT according to GOÄ. The cost of breast augmentation with autologous fat is around €6,800 including VAT according to GOÄ.
Health insurance companies only cover the costs of breast enlargement surgery if there is a medical necessity. A corresponding indication for breast augmentation with implants is only given in exceptional cases, e.g., for plastic reconstruction of a breast after surgery for cancer treatment, for breast aplasia (congenital absence of the breast), or for transgender patients (e.g., for male-to-female breast conversion).
Breast enlargement with implants
Breast enlargement with implants at HANSEATICUM Hamburg combines the highest medical standards with the wishes of our patients to achieve natural-looking aesthetic results. Our experienced specialists use only FDA-approved (American Food and Drug Administration) or CE-certified breast implants of the latest generation. These are filled with a high-quality, thick silicone gel and have a double-walled shell that can be either smooth or textured. The implants are available in various shapes—round or teardrop—and can be adapted to the individual needs and anatomical conditions of the patient. The choice of implant and its positioning, whether above or below the chest muscle, depends on many factors. These include the patient's body structure, lifestyle, and aesthetic preferences. As a rule, access for implant insertion is gained via an inconspicuous incision in the crease under the breast, which minimizes the risk of visible scarring.
The operation takes about 1 to 1.5 hours and can be performed under general anesthesia. Although the procedure can often be performed on an outpatient basis, we usually recommend an overnight stay for optimal monitoring and aftercare. After the operation, it is necessary to wear a special compression bra to support the new shape of the breasts and promote healing. Sports activities should be paused for about six weeks to ensure the best possible result.
Breast augmentation with Motiva® implants
HANSEATICUM in Hamburg is one of the first clinics in Germany to be authorized to use Motiva® Ergonomix2® breast implants. This generation of implants differs from conventional breast implants in several ways: The filling moves with every movement and allows the implants to look natural whether lying down or standing up. Although they are round in shape, they behave like anatomical implants.
The surface is finely structured instead of classically textured. In current studies, highly textured implants are suspected of promoting certain complications. The smoother surface of Motiva implants is designed to minimize this risk. A patented barrier layer also gives the implants a bluish sheen, making even the smallest damage to the shell visible.
The multi-layered structure also makes the implants extremely flexible. “This makes it possible to insert the implants through incisions of only 2.5 to 3.5 centimeters, which leave correspondingly less noticeable scars,” says Dr. Cornelius Grüber. The minimal scar technique requires special expertise, which our specialists have thanks to their many years of experience.

B-Lite implants: An easier alternative?
Breast enlargements have been number one on the popularity scale for cosmetic surgery for years. In the future, it could be even easier for women to decide to have them: new ultra-light breast implants weigh about a third less than conventional implants.
The B-Lite implants were developed in Israel and are now manufactured by Polytech in Germany. “Logically, there are no long-term studies yet, and the CE-certified B-Lite implants are not yet available for operations covered by health insurance. However, their technical data are convincing, and our experience with products from this manufacturer has been consistently positive,” says Dr. Cornelius Grüber.
While the shell of the new breast implants is made of the same proven material as brand-name implants, the filling consists of a specially prepared silicone gel containing microscopic hollow spheres. Thanks to these microspheres, which are so securely fixed that they cannot migrate, B-Lite implants have the desired volume but weigh much less—a 300-milliliter implant that previously weighed approximately 300 grams now weighs just under 220 grams with the new technology. The technology is based on findings from NASA space research on the production of materials that are lightweight but otherwise have the same properties.
BREAST ENLARGEMENT WITH AUTOLOGOUS FAT
Breast enlargement with autologous fat, also known as lipofilling, offers a natural and gentle alternative to traditional silicone implants. This method is ideal for women who want moderate breast enlargement or to correct minor asymmetries. The body's own fat, usually from areas with excess fat deposits such as the abdomen, hips, or thighs, is removed and injected into the breast. This process does not require large incisions, only small punctures that are placed so that they are barely visible after healing.
The procedure can be performed under twilight sedation or general anesthesia, depending on the patient's preferences and health situation. After removal, the fat is prepared and evenly injected into the breast using special cannulas. The distribution of the fat is carefully planned to ensure a harmonious and natural appearance. Since fat cells are very sensitive, it is important that no pressure is exerted on the breast after the operation and that it is protected from the cold to ensure the survival of the transplanted cells.
Approximately 70 to 80 percent of the transplanted fat cells survive permanently, resulting in a long-lasting enlargement effect. An additional advantage of this method is that the breasts behave naturally after weight gain and can grow along with it. Although the size of breast enlargement with autologous fat is limited to half to one cup size per session, further treatments can be performed to achieve the desired volume.
Breast enlargement with implants or autologous fat?
The majority of women still opt for breast enlargement with implants, but breast enlargement with autologous fat is definitely gaining popularity. “Lipotransfer for breast augmentation is ideal for women who want a subtle, particularly natural-looking result or want to correct asymmetry,” says Dr. Sonja Tomschik, who also successfully uses autologous fat for reconstructive surgery after breast cancer.
The body's own filling material is removed from excess fat deposits on the abdomen, hips, or thighs using liposuction and injected directly into the breast, where it mostly grows in permanently. Since only small punctures are necessary for the cannulas, no scars remain.

Breast enlargement with minimal scar technique
“Minimal scar” means “minimal scarring.” In plastic surgery, the use of scar reduction techniques has been common for years, for example in breast reductions or mastopexies (breast lifts). With the development of new implants and advanced surgical instruments, it is now possible to perform breast augmentation using the minimal scar technique. This approach allows for an incision in the breast fold that is only 2.5 to 3 cm long, about half the length of traditional breast augmentation methods. When will my breasts settle after breast enlargement surgery?
After breast enlargement with implants, the breasts will feel tight and firm for several weeks. Gradually, their shape and firmness will change and the implants will settle. It can take up to a year for the final result to be visible. However, if you feel that one breast is not relaxing or is curving downward, causing the nipple to point downward, you should contact your plastic and aesthetic surgeon.
WHICH IMPLANT SIZE IS RIGHT FOR ME?
A few years ago, B was the most requested cup size, but now the trend is more towards a C cup. In practice, women's wishes and ideas vary greatly. While one woman feels comfortable with a natural-looking B-C cup, another dreams of significantly more volume. It is important to have a detailed consultation with your specialist to determine the desired and appropriate size and shape. Your lifestyle should also be taken into account in the decision, as well as your individual physical characteristics, including the width of your back (a larger implant is recommended for a broad back), the amount of fatty tissue present, and the elasticity of your connective tissue.
HOW LONG DO BREAST IMPLANTS LAST?
For a long time, there was a rumor that breast implants had to be replaced every 10 years. This is incorrect, as a study by the US Food and Drug Administration (FDA) has shown. Modern implants cannot leak, have no expiration date, and can therefore last a lifetime, depending on the individual's lifestyle. In some cases, increased capsule formation or changes in the shape of the breast may necessitate replacement.
BREAST ENLARGEMENT ABOVE OR BELOW THE MUSCLE?
Breast enlargements with implants under the muscle have a low risk of developing capsular fibrosis. This involves the formation of a hardened connective tissue-like capsule around the implant, which can be painful and deform the breast - and can only be remedied by replacing the implant. Under the muscle, there are no visible wrinkles or ripples that can occur when moving the arms. The breasts also look more natural and feel softer when the implant is placed under the chest muscle. Disadvantage: The fairly strong muscle limits the size of the implant. And movements of the upper body can cause the implant to shift, making it temporarily visible. However, this mainly applies to women who are very active in sports.
If the implants are placed above the chest muscle during breast augmentation, the volume can be significantly larger. Women with weak connective tissue and a lot of soft tissue provide optimal embedding for the implant – and very beautiful results. This position is also often better for athletic women who want to avoid implant deformation due to muscle contraction. Disadvantage: Deformations tend to be more visible from the outside. However, if there is sufficient soft tissue coverage, any change in the shape of the implant can also be adequately concealed. Implants placed above the muscle, especially very large and shaping implants, are slightly more palpable - however, the viscosity of implants has improved so much in recent years that positioning above the muscle is very popular.
IS MAMMOGRAPHY POSSIBLE WITH BREAST IMPLANTS?
Traditional silicone implants can make mammograms difficult, as the breast tissue near the implant often cannot be compressed sufficiently. However, there are now examination methods that are tailored to implants.
In a conventional mammogram, about 4 percent of the breast tissue cannot be visualized well after breast augmentation with implants, which means that more images of the breast have to be taken during a screening exam. Ultrasound or MRI offer a useful alternative here, with more accurate results.
WHAT ARE THE FIRST DAYS AFTER THE OPERATION LIKE?
A feeling of tension, varying in intensity, during the first few days after the procedure is completely normal. Severe pain is less common. The following can help: a good painkiller and measures to reduce swelling. The scar is covered with a waterproof plaster, which means you can shower again after just one day.
HOW LONG DO I NEED TO TAKE IT EASY AFTER BREAST AUGMENTATION?
Depending on your occupation, you should expect to be unable to work for between 7 and 14 days and avoid physical exertion. The healing process varies in duration. The breast is usually only fully resilient again after six to twelve weeks.
WHAT SHOULD I BE AWARE OF AFTER BREAST ENLARGEMENT SURGERY?
Behaving correctly after the operation makes an immensely important contribution to the healing process and thus has a decisive influence on the outcome!
The most important thing: attend all postoperative appointments at HANSEATICUM for checkups and wound care so that any complications that may arise can be detected and treated at an early stage. Pay attention to whether your breast hardens, changes position, or changes in color or temperature. Pain, secondary bleeding, and sensory disturbances are serious signs that occur very rarely but should be reported to your specialist immediately.
If possible, refrain from smoking and drinking alcohol for two weeks after breast augmentation so as not to interfere with the wound healing process.
Be sure to leave the removal of the bandage and drains to your specialist!
In the first few days, a support bandage prevents the implants from slipping. After that, a special bra is recommended: day and night for 6 weeks, then another 6 weeks at night and during exercise. It is essential that you leave the bra in place. Do not change the individual fit that your doctor has adjusted.
Arm movements are uncomfortable at first. For the first six weeks, do not support your arms, do not make any quick upward stretching movements, do not lift anything heavy - only moderate movements. Scar care with special oil products or silicone plasters is recommended. The surgical scars should be protected from direct sunlight for approximately 6 months.
WHEN CAN I START PLAYING SPORTS AGAIN AFTER THE OPERATION?
This varies greatly from person to person and also depends on the type of sport and the type of breast augmentation. As a rule, it takes at least 6 to 10 weeks, as the healing process is different for every woman. Be sure to talk to your specialist before you start jogging or going to the gym again.
CAN YOU STILL BREASTFEED AFTER BREAST AUGMENTATION?
